
Serious Illness Conversations Workshop
OVERVIEW
This 1.5-day workshop will equip healthcare professionals with the skills to conduct serious illness conversations with patients and families. The Serious Illness Conversation Program offers healthcare professionals a structure and language to talk with seriously ill patients about their goals, values and priorities as part of shared decision making
Learning Objectives:
- Appreciate the importance and benefits of serious illness conversations.
- Discuss the evidence for the Serious Illness Conversation (SIC) Program
- Identify the key elements of the SIC Guide
- Practise use of the SIC guide
- To understand the similarities and differences between SIC and Advance Care Planning
- Discuss the barriers and enablers to real world implementation of the SIC Program

e-CERTIFICATE OF COMPLETION

Doctors: SGD$350
Nurses, MSWs and Other Healthcare Professionals: SGD$200
All fees indicated are before GST

• This workshop is pending accreditation from the Singapore Medical Council, Singapore Nursing Board & Singapore Pharmacy Council.
• Continuing education credits are available for LCPC events through the Singapore Medical Council (SMC), Singapore Nursing Board (SNB) and Singapore Pharmacy Council (SPC) subject to their respective approvals.
Upon completion of each workshop, participants can claim up to 6 CPE points from SMC/SNB or up to 5 CPE points from SPC.
Attendees who wish to avail of the credits are required to sign the relevant registration form along with their CPE/CME number on the day(s) of the events.
The attendee’s registration details will be submitted after the event and credited upon receiving respective approvals.
